In an effort to determine the memberships' interest in events and activities, we ask that you download the PDF, fill out the questionnaire, and submit it to the web manager.
When you download the questionnaire, the form may be displayed by your web browser. In this case, you should be able to make the appropriate entries and save the updated file (PDF) on your computer. If this doesn't work, download and save the file to your computer. Next make the appropriate entries in the form by opening the file in your browser. Once all the information has been entered, save the file. Go back to the member interest page. Attach the modified file to the "COMPLETED QUESTONNAIRE" form below and click "SUBMIT QUESTIONNAIRE"

DownloadAfter you have completed the questionnaire, provide an e-mail address, attach the file below. You may also add a message, if you desire. When complete submit the questionnaire.
